ELIZABETHTOWN — The Essex County Office for the Aging will host a six-week workshop that focuses on living healthy with chronic conditions.

The workshops will be done by telephone with a trained facilitator. Workshops will be held each Tuesday from 1 to 2 p.m., starting May 18.

The workshop will include evidence-based self-management skills and tools to help participants learn how to manage the physical and emotional toll of their chronic conditions.

Each of the participants will be provided with a tool kit to help them become active self-managers of their conditions through problem solving, decision making, goal setting and action planning.
